Upload records you already have
Use the clinic’s secure file-transfer service to send relevant laboratory reports, imaging results, or medical records. Upload only records that relate to the care you are seeking.
If you prefer, contact our admin team about a signed Release of Records authorization. With your permission, we can request a copy of your most recent relevant records directly from your family doctor’s office.
Your doctor’s office can send the records to us through its secure records-release process.
