Lifestyle and clinical context

Build a useful picture before choosing a next step

We begin with the foundations that support overall and metabolic health: regular movement and strength exercise, restorative sleep, nourishing food, recovery, and attention to weight and insulin resistance when relevant to your symptoms. These are encouraged before testosterone therapy is considered, especially when excess weight or metabolic factors may be contributing.

Fatigue, low mood, reduced libido, changes in strength, and difficulty concentrating can have many possible causes. Sleep disruption, stress, medications, mood, metabolic health and other conditions may overlap with symptoms commonly attributed to low testosterone. Addressing relevant factors may improve symptoms for some men, though results vary. A careful assessment helps identify possible contributors and prioritize overall health before deciding whether TRT is appropriate.

Testosterone deficiency is assessed using symptoms and signs together with appropriately timed testing; a low result is generally confirmed with repeat morning testing. The tests selected depend on your history, symptoms and initial findings. Examples may include:

Hormone assessment

Total testosterone, with SHBG and an appropriate free-testosterone assessment when indicated. Estradiol or dihydrotestosterone (DHT) may be considered when the clinical question supports them. A Free Androgen Index may appear on a lab report, but it is not a stand-alone diagnosis or a substitute for an appropriate free-testosterone assessment.

Metabolic health

Fasting glucose, fasting insulin and HbA1c. A HOMA-IR estimate may be calculated from fasting results when it is useful in context.

Other health markers

A lipid profile and liver and kidney function. Lipoprotein(a), hs-CRP or ESR may be considered when your symptoms, history or risk profile support them.

This is an example list, not a standard panel. Not everyone needs every test. If treatment is considered, additional safety checks may include a blood count and hematocrit, fertility discussion, and age- and risk-appropriate prostate assessment.

Our approach is measured. TRT is not used to prevent normal age-related changes or to push testosterone above the normal physiologic range. When a diagnosis is confirmed and treatment is appropriate, the aim is to improve relevant symptoms while keeping levels within a physiologic range, with careful dose adjustment and monitoring. Side effects remain possible, and TRT can be a long-term commitment that suppresses sperm production and affects fertility. Before starting, discuss fertility plans, likely benefits, risks, alternatives and follow-up with your clinician.

Your plan is individual. Lifestyle support is part of care from the start. If symptoms continue, an appropriate assessment can help determine whether further testing or treatment options should be discussed.
